At the same time, elevated hematocrit is frequently mismanaged - either ignored when it warrants attention, or panicked over when it doesn't.

What hematocrit is and why TRT raises it

Hematocrit is the percentage of your blood volume made up of red blood cells. A normal range for men is roughly 40-52%, though lab reference ranges vary slightly. Testosterone increases red blood cell production by stimulating erythropoietin (EPO) production in the kidneys - the same mechanism athletes attempt to exploit with EPO doping. This is an expected pharmacological effect of TRT, not an unusual reaction.

Most men on TRT see hematocrit rise 3-6 percentage points from their pre-TRT baseline within the first 6-12 months. Where it stabilizes depends on dose, injection frequency, hydration, altitude, sleep apnea status, and individual variation.

When elevated hematocrit becomes a concern

The concern with high hematocrit is blood viscosity. Thicker blood is harder to pump and increases the risk of clotting events - deep vein thrombosis, pulmonary embolism, stroke. Most guidelines use 54% as a threshold where intervention should be considered, though some clinicians act at 52-53% in men with other cardiovascular risk factors.

It's worth noting that elevated hematocrit in athletes (who have high red blood cell production from training adaptation) seems to carry different risk than elevated hematocrit from polycythemia or exogenous hormone use. The population data on TRT-specific hematocrit risk is still evolving. But taking high hematocrit seriously is appropriate - the mechanism for harm is real even if the magnitude of risk in healthy TRT users is debated.

Symptoms of hematocrit that's too high

Many men with elevated hematocrit are asymptomatic. But some notice:

  • Headaches, particularly in the morning
  • Flushed face or skin
  • Fatigue and reduced exercise tolerance
  • Sleep disruption, including worse sleep apnea symptoms
  • A heavy or "thick" feeling

If you're experiencing any of these and haven't checked hematocrit recently, that's the first place to look.

What to do about elevated hematocrit

The primary interventions are dose reduction, frequency adjustment, and therapeutic phlebotomy (blood donation or therapeutic bloodletting). Each has tradeoffs:

Dose reduction is the cleanest fix if the dose is higher than necessary. If hematocrit is high and symptoms are suboptimal, a modest dose reduction sometimes solves both problems.

Frequency adjustment can help because more frequent, smaller doses produce smaller testosterone peaks, which may drive less erythropoiesis. Some men see meaningful hematocrit reductions by switching from weekly to twice-weekly injections at the same total dose.

Therapeutic phlebotomy - donating blood or having it drawn - reduces hematocrit quickly. Regular blood donation (every 8 weeks at most donation centers) is a commonly used management strategy. The limitation is that it's a recurring intervention rather than addressing the underlying driver, and repeated phlebotomy can cause iron deficiency over time.

Hydration matters more than people think Hematocrit is a ratio - red blood cells as a percentage of total blood volume. Dehydration contracts plasma volume and artificially elevates hematocrit without actually increasing red blood cell count. Checking hematocrit when you're dehydrated can produce a misleading result. Always be well-hydrated before a lab draw, and if a hematocrit result seems surprisingly high, recheck it under better conditions before making protocol decisions based on it.

Sleep apnea and hematocrit

Untreated sleep apnea is an independent driver of elevated hematocrit. Repeated episodes of low oxygen during sleep stimulate red blood cell production as a compensatory response. TRT can worsen sleep apnea in susceptible men, which then further elevates hematocrit - creating a compounding effect. If your hematocrit is rising despite dose adjustments and adequate hydration, and you have any sleep apnea symptoms, an evaluation for sleep apnea is warranted.

Monitoring frequency

Most clinicians check hematocrit every 3-6 months on TRT. In the first year, more frequent monitoring (every 3 months) is appropriate because this is when the rise is most rapid. Once it stabilizes in an acceptable range, twice-yearly checks are reasonable for most men.

Don't skip these labs because you feel fine. Elevated hematocrit is often asymptomatic until it's significantly high, and it's one of the few TRT-related labs where the risk profile justifies consistent monitoring even in the absence of symptoms.
